OUR VISION
Ushering a world where no patient deteriorates unseen between moments of care.

“Everyone deserves continuous, connected care.”
CareCentra began with my father. He was a 90-year-old patient living in India with congestive heart failure and a respiratory illness. He wouldn't weigh himself every day. He knew it could help spot the need for diuresis early and prevent an ER visit. But he saw it as the first step toward prolonged hospital stays.
Then something changed. I placed a connected weighing scale by his bedside that built a line graph on his iPad each time he stepped on it. The nudge worked! Every evening at 7:30 PM in New York, my phone would ping. It was 6 AM in India, and my 90-year old father had stepped on a connected scale.
Dad never knew his actual weight, nor did he care. Yet, in the early days, there were many pings in a day as he built that graph with childlike wonder. But over six years, wonderment gave way to routine, and the habit kept him stable. He didn't visit the ER. He went in for preventive diuresis only twice during that time - a simple process.
For me, one daily ping became his heartbeat from 10,000 miles away. Until there were none. He passed away in his sleep shortly after he turned 96.
My mission has been to replicate that remote, connected monitoring at scale. We aim to have the doctor's office reach the patient before they experience a flare up. It is the human voice and touch that is magical. We make it possible to reach out to patients even before they know they need it.
Put a smile on an aging face that looks up from a device and says, “What would I do without you?” It is priceless!

WHAT WE STAND FOR
Our Principles
These are the unchanging principles that guide every decision we make and every product we build.
Patients First
CareCentra exists because of patients like Vasant's father. Every decision we make, every feature we build, starts with one question: will this help keep patients healthier? Patient outcomes drive everything we do.
OUTCOMES OVER EVERYTHING.
Machine Intelligence + Human Compassion
Technology should amplify human care, not replace it. Our AI handles routine monitoring so care teams can focus on what matters most — the complex interventions that require clinical judgment and human connection.
AI THAT MAKES CARE MORE HUMAN.
Proven Partnership
We only succeed when our health system partners succeed. That means measurable outcomes, sustainable economics, and seamless integration. We're in this together for the long term.
YOUR SUCCESS IS OUR SUCCESS.
THE PEOPLE BEHIND THE PLATFORM
We Aim To Treat Who The Patient Is Becoming
We assembled a multidisciplinary team to work with software engineers in an offshore Knowledge Center over six years to codify patient behaviors and nudges, clinical workflows, and UX designs from three clinical trials and two multi-year programs, and algorithms that predict rising risk to deliver the first version of our connected care solution

Our Mission
Healthcare is fundamentally human. At its core, it's the business of humans helping other humans live well for as long as they can.
But too often, patients with chronic conditions fall through the cracks. They're seen for 15 minutes every few months, then left to manage complex conditions on their own. By the time symptoms escalate, they end up in the emergency room.
We believe there's a better way. Continuous monitoring that catches problems early. Predictive AI that identifies risk before symptoms escalate. Human care teams that intervene at exactly the right moment.
This is the motive behind CareCentra — to wield technology not as a means to replace the humanity of healthcare, but to amplify its impact. To extend its reach. To ensure that every patient has a care team watching over them, ready to help whenever they need it.
The DNA of CareCentra is making healthcare more human.

“These aren't incremental improvements. They represent a fundamental shift in how respiratory populations can be managed.”
